JOB ANNOUNCEMENT – COORDINATION OF CARE SPECIALIST
Common Wealth Development is seeking a motivated individual to implement a comprehensive care model to support our program participants and housing tenants and advance our racial justice and health equity work. The Coordination of Care Specialist will be responsible for assisting Common Wealth’s Housing and Adult Workforce Development sections by supporting our efforts in West Madison to stabilize housing, connect individuals to employment opportunities, and to provide connections to other available resources. This is a full-time salaried position with benefits, including a generous paid leave package and health insurance. Salary is $35,000 - $40,000, depending on experience. This is a grant-funded position; unless Common Wealth receives additional funding, the term of the position is linked to the term of the grant. Applicants should have a valid driver’s license and regular access to an insured car.
